PCOS Hair Fall: How to Stop It and Which Tests to Ask For

Short answer: PCOS (polycystic ovary syndrome) can cause scalp hair thinning because of higher androgen (male hormone) activity. It usually shows as a widening parting or thinning on top, rather than bald patches. It's managed, not cured, with medical treatment through a gynaecologist, endocrinologist or dermatologist, together with lifestyle changes. Shampoos and oils alone won't stop it.

How PCOS affects hair

PCOS is a common hormonal condition. Many women with PCOS have higher androgen levels, or follicles that are more sensitive to androgens. On the scalp, androgens can shrink follicles over time (female pattern hair loss). On the face and body, the same hormones can cause extra hair growth (hirsutism). That's why some women with PCOS notice both thinning on the head and more hair on the chin.

Other PCOS signs include irregular or missed periods, acne, weight gain or difficulty losing weight, and darkened skin folds (acanthosis nigricans). Not every woman has every sign.

What PCOS hair loss looks like

  • A wider parting (maang) at the top of the head
  • Thinner ponytail volume
  • More scalp visible on top, with the front hairline usually kept
  • Gradual change over months to years

Sudden heavy shedding all over the head is more often telogen effluvium (after illness, delivery or stress), although women with PCOS can have both.

Tests to ask your doctor about

Your doctor will choose the right tests, but common ones include:

TestWhy
Testosterone (total or free), sometimes DHEA-SChecks androgen levels
LH, FSH, prolactinHormone pattern and other causes of irregular periods
Thyroid (TSH)Thyroid problems also cause hair loss and period changes
Ferritin, CBCLow iron makes shedding worse and is common in Pakistan
Vitamin DOften low and easy to check
Fasting glucose or HbA1c, lipidsPCOS is linked with insulin resistance
Pelvic ultrasoundSometimes used as part of diagnosis

How PCOS hair fall is managed

Medical options (with your doctor)

  • Topical minoxidil is commonly used for female pattern hair loss. It is not suitable during pregnancy or breastfeeding.
  • Hormonal treatments, such as certain combined oral contraceptives or anti-androgen medicines, are sometimes prescribed. These need medical supervision, and some must not be taken if you could become pregnant.
  • Treating insulin resistance. Your doctor may suggest medicines that help with insulin and cycle regularity.
  • Correcting iron or vitamin D deficiency, if your tests show one.

Results take months. Hair grows slowly, and hormonal treatments typically need six to twelve months to show their effect on the scalp.

Lifestyle steps that genuinely help

  • Gradual weight loss, if you are overweight. Losing even a modest amount can improve hormone balance and periods for many women.
  • Regular activity: brisk walking, strength exercise, anything you'll keep doing.
  • Balanced meals: more protein and fibre, fewer sugary drinks, less mithai and maida.
  • Sleep and stress.
  • Avoid crash diets. Rapid weight loss causes its own shedding.

What won't stop PCOS hair fall

  • "PCOS shampoos" that claim to block hormones
  • Oils, onion juice and other totkay
  • Unverified supplements and "hormone balancing" teas
  • Products promising regrowth in weeks

A gentle shampoo is still useful. It keeps the scalp clean and treats dandruff if you have it. It just can't fix a hormonal cause.

Styling while you wait

  • Try a side parting, or a zig-zag parting, to soften a wide maang.
  • Layered cuts add volume.
  • Choose loose hairstyles over tight pulls.
  • Hair fibres or a tinted dry shampoo can disguise thinning on special occasions.

Feelings matter too

Hair changes, along with acne or facial hair, can hit self-esteem hard, especially with family comments or rishta pressure. You are not alone, and PCOS is extremely common. A supportive doctor and realistic plan make a big difference.

Quick answers

Will my hair come back once my periods are regular? Shedding often improves as hormones settle, but follicles that have thinned need time and sometimes treatment. Expect months, not weeks.

Is spearmint tea worth trying? Small studies looked at spearmint tea for facial hair in PCOS, with limited results. It's generally safe as a drink, but it isn't a treatment for scalp thinning.

See a doctor if…

  • You have irregular periods along with hair thinning or extra facial hair
  • Your parting is widening over months
  • Hair loss is sudden or patchy
  • You're trying to conceive and want to know which treatments are safe
  • You have symptoms of high blood sugar, such as excessive thirst or frequent urination

Bottom line

PCOS hair fall is hormonal, so the fix starts with a diagnosis and a medical plan, supported by gradual lifestyle change. Ask for the right tests, be patient for six to twelve months, and skip products that promise to "balance hormones" through your scalp. Also read thyroid and hair loss.
